EMERGENCY GRANTS FOR INDIVIDUALS
The Fazel Family Foundation provides one-time grants to individuals faced with a medical crisis.
The Fazel Family Foundation provides:

Medical emergency and distress grants for those facing a serious or life-threatening illness or terminal condition with likelihood of prolonged hospitalization and/or intensive medical care. The Foundation will also consider requests for emergency relief to assist with a patient's short-term, immediate inability to pay for medical care, such as insurance co-pays and specialty medical equipment.

One-time donations of up to $5,000 within a 12-month period
Third-party verification from a health care provider is required.
